The Employee Benefits Security Administration (EBSA) announced the availability of the 2011 Form M-1 annual report for multiple welfare arrangements (MEWA). In December 2011, EBSA proposed revisions to the form and instructions. All 2011 filings (MEWAs and Entities Claiming Exception) should use the year 2011 Form M-1.
Filing deadline is March 1, 2012, however, administrators can request an automatic 60-day extension until May 1, 2012.
